Health care can be responsive to whole-person health needs.

CHaSCI advances the practice change needed to break down barriers to health by training social workers, community health workers, care managers, and navigators on our CHaSCI social care model and core competencies for integrating health and social care.

 
 
 

CHaSCI’s social CARE MODEL

Since 2008, CHaSCI has innovated to develop effective models of care that leverage social workers and community health workers to improve quality of care and ultimately improve health and health equity.

CHaSCI’s social care model is grounded in our history developing, testing, and disseminating two social work-led care management models:

  • The Bridge Model of transitional care and

  • The AIMS Model (Ambulatory Integration of Medical and Social)

Our CHaSCI social care model has evolved based on on-the-ground experience and implementation research. Our impact findings include:

  1. Increased engagement with primary care

  2. Lower hospitalization and readmission rates

  3. Decreased depressive symptoms and lower stress for patients & family caregivers

 
 

 
 

Care model training and technical assistance

CHaSCI provides hands-on training and technical assistance to organizations looking to implement our CHaSCI social care model or broader core competencies for health and social care integration.

Since 2008, we have partnered with over 200 organizations to provide training and guidance so they can build on our evidence base and experience rather than starting from scratch. We also provide knowledge- and skill-based training for Community Health Workers (CHWs) and other interprofessional providers to equip people with practical knowledge base and applicable skills for interfacing with and caring for diverse community members.

We have standardized training packages as well as tailored approaches to meet sites’ unique needs and preferences.
