+ Based at Rush University Medical Center

  • CHaSCI is housed at Rush University System for Health, a large academic health system on the west side of Chicago. We are run by the Department of Social Work and Community Health.

+ Working in strong collaboration with others

  • CHaSCI's care models Bridge and AIMS have been developed collaboratively with partners from across Illinois and tested in diverse communities and systems. Our care model partners include aging network community-based organization Aging Care Connections and public health leader from Health & Medicine Policy Research Group.
  • Today, we collaborate actively with numerous local and national partners on care model research, policy advocacy, and communications development - including via the National Coalition on Care Coordination and the Coalition for Social Work and Health
